How to Get a Quote
Step 1 – Go to the MetLife dental insurance page (click here – link will open in a new browser window/tab).
Note: If you are enrolled in a Tricare plan (TDP), go to the ‘TRICARE Employee Benefits’ sign-in page (link opens in new tab).
Step 2 – Press the ‘Sign in to ‘MyBenefits’ link. You will be taken to the main account access page.
Step 3 – Enter the name of your company and hit ‘Submit.’
Step 4 – Follow the on-page instructions to gain access to your group insurance policy information. Contact your employer if you require certain details in order to gain access to your policy.
Make a Claim
Step 1 – On the main dental page, click the ‘Download Claim’ link to download a .PDF file of the employee claim form.
Step 2 – Print the form out and fill out all required fields and information sections. After filling out the entire sheet and signing your signature in the required areas, give the form to your employer’s department that processes benefits claims.
