If you’re a current Aflac disability insurance policyholder, learn how to access your policy online by following the step-by-step instructions in the guide below. How to Log In
Step 1 – Go to the individual insurance homepage by right-clicking the login button at the top of this page (and loading the page in a new window/tab), or by clicking here (link opens in a new window).
Step 2 – Press the ‘Log In’ link and then select ‘Individual.’
Step 3 – Enter your user ID and password. Click the ‘Login’ button to access your account.
Forgot Password – On the main login page, click the ‘Forgot Password’ link. Enter your user ID and then hit ‘Submit.’ Continue the process until your identity has been verified and you are able to reset your password.
Enroll in Online Access
Step 1 – On the homepage, press the ‘Register’ link in the upper-right corner. Select ‘Individual.’
Step 2 – Press the ‘Register Now’ link.
Step 3 – Enter your policy number and then click ‘Submit.’ Enter your personal details, and then create your account credentials (user ID, password, security verification details).
Make a Claim
Short-term disability claims should be handled by your employer. You can find specific claim forms on the main claims page.
Step 1 – Go here (link will open in new window).
Step 2 – Select your state from the drop-down list, and then click the claim form you want to download. The form will download as a .PDF file.
