Click the ‘Next’ button to continue. Enter your payment information, verify the details, and then confirm your payment to complete the process. Report a Claim Step 1 – Go to this page. The online claim submission form is four steps long. You will need to have your policy number, personal information, and incident information on-hand in order to successfully submit your claim.
