Step 4 – Read and agree to the terms and conditions and click ‘Continue’. Step 5 – Enter your policy information. This includes the policy number, last 4 digits of SSN/TIN, date of birth and date of issue. Click ‘Continue’. Step 6 – Enter all required information to complete the enrollment process.
