Emblem Health Insurance Login | Make a Payment

Sign up for Emblem Health Insurance online access to review policy/benefit/copay information, check the status of claims, pay your premium, view/print your ID card, and change your primary care provider. The guides below detail the enrollment, sign-in, password reset, and claims processes.

Note: You can access your policy via the mobile app as well (download links for iOS/Android are at the top of this page).

How to Log In

Step 1 – Go to the homepage (http://www.emblemhealth.com/) and then click the sign in link in the upper-right corner of the page.

Note: You can click the login button at the top of this page to go directly to the main sign-in page.


Step 2 – Enter your user ID and password into the pop-up login form, and then click the sign-in button.


Forgot Password – Click the “Forgot User ID/Password” link on the sign-in form/page. Enter your date of birth and member ID, and then click the “Validate” button. After your identity has been validated you will be able to reset your password/PIN.


Enroll in Online Access

Step 1 – The registration page can be reached by clicking the “register for access” link on the sign-in page/form.


Step 2 – Enter your date of birth/member ID and hit the validate button. Provide your first name, last name, email address, chosen username/password/security question, and then hit the submit button.


Make a Payment

Step 1 – Online payments can be made by signing in to your account.

File a Claim

Step 1 – Check the status of an open claim by signing in to your account on the website or mobile app. If you need to file a claim, go to the forms page and then select the appropriate claim form to download/fill out.
