Fidelity Life Insurance does not currently have an online policy management tool. If you’re a current policyholder wondering how to pay your bill, make changes to your policy, or file a claim, read the instructions in the sections below.
How to Make a Payment
Step 1 – Payments can be made via credit/debit card or electronic funds transfer. Premiums can be paid on a monthly, quarterly, semi-annual, or annual basis. If you want to make changes to your payment method/details, you need to contact your independent agent or get in touch with FLA’s customer service line (800-369-3990).
Note: In order to set up or change your payment method you may need to fill out the “premium payment option” which can be found on the service forms page (link opens in a new window/tab).
File a Claim
Step 1 – Go to the claim forms page here and then select your/the policyholder’s state of residence from the list of options.
Step 2 – Download the form and then print it out. Enter the following information into the form:
- Policy number
- Decedent information (name, residence, date of birth, place of death, date/cause/manner of death)
- Beneficiary/claimant information
- Payment of fund (single sum, installment)
Step 3 – The death claim form must be notarized before it can be submitted for processing. After the form has been received by FLA you will be contacted by a representative.
