How to Get a Quote

Step 1 – Starting on the Multiflex homepage, click the ‘Get a Quote’ button as outlined in the screenshot below.


Step 2 – You will be transferred to the quote page. Enter or select your ZIP code, age group, coverage type and effective date. Click the ‘Next’ button.


Step 3 – Review the different dental plans. It’s important to closely compare the various options and benefits that each plan provides. Once you have selected which plan you want to receive a quote for, click the ‘Select Plan’ button under that plan’s column.


Step 4 – Select your preferred coverage term from the drop-down menu. Review the plan details that appear to the right of the menu, and then click the ‘Next’ button.


Step 5 – The next section is titled ‘Insured Coverage Information.’ Enter your personal details (full name, SSN, DOB, address, email, gender, online account credentials, etc.) into the input fields. Press ‘Next’ at the bottom of the page.
