Step 1 – Starting on Look’s homepage (http://lookinsuranceagency.com/), click the “Quotes” link in the main menu (outlined in red in the below screenshot).


Step 2 – Once on the main quotes page, select which type of policy you want to receive a quote for (for example purposes this guide will detail how to submit an auto policy request form).


Step 3 – Enter your first name, last name, email address, phone number, and ZIP code into the input fields. Click the “Start” button at the bottom of the form to begin the actual process (you will be transferred to the four-step form).


Step 4 – Enter the following information into the form:

  • Address
  • City
  • State
  • ZIP
  • DOB
  • Occupation
  • Education
  • SSN
  • Consent to credit report (yes/no)
  • Residence type
  • Residence ownership
  • Gender
  • Marital status


Step 5 – After reviewing the information you entered, hit the “Proceed” button to move to the next step. In the following steps, enter as much of the required information as possible in order to receive an accurate quote.
