Use the free Emblem Health Insurance quote tool (on the EmblemHealth) website to receive an accurate estimate of how much a health plan will cost you, your family, or your business.  Read the guide in the section below for an overview of the quote process. Note: Policies are available to residents/businesses of New York. How to Get a Quote Step 1 – The first step is to go to the homepage (http://www.emblemhealth.com/). Step 2 – Hover over the “Our Plans” button and then select which type of policy you want to purchase (individual/family, small business, large business, medicaid, medicare). Step 3 – Scroll down and then click the “Get My Quote” link. Note: You can also search for available policies in your area by entering your ZIP code into the search bar and then clicking the “See Your Plans” button. Step 4 – Enter your first/last name, date of birth, ZIP code, and then answer whether or not you have a medicare card. If you want to add a member of your family to the policy, click the “Add Member” button and then enter their details. Press the “Next Step” button to go to the next section. Step 5 – Enter your household/income details. After providing this information you will be presented with your rate information. Note: Due to the Affordable Healthcare Act, most policies can only be purchased during an open enrollment period (unless you qualify for an exemption).

Sign up for Emblem Health Insurance online access to review policy/benefit/copay information, check the status of claims, pay your premium, view/print your ID card, and change your primary care provider. The guides below detail the enrollment, sign-in, password reset, and claims processes. Note: You can access your policy via the mobile app as well (download links for iOS/Android are at the top of this page). How to Log In Step 1 – Go to the homepage (http://www.emblemhealth.com/) and then click the sign in link in the upper-right corner of the page. Note: You can click the login button at the top of this page to go directly to the main sign-in page. Step 2 – Enter your user ID and password into the pop-up login form, and then click the sign-in button. Forgot Password – Click the “Forgot User ID/Password” link on the sign-in form/page. Enter your date of birth and member ID, and then click the “Validate” button. After your identity has been validated you will be able to reset your password/PIN. Enroll in Online Access Step 1 – The registration page can be reached by clicking the “register for access” link on the sign-in page/form. Step 2 – Enter your date of birth/member ID and hit the validate button. Provide your first name, last name, email address, chosen username/password/security question, and then hit the submit button. Make a Payment Step 1 – Online payments can be made by signing in to your account. File a Claim Step 1 – Check the status of an open claim by signing in to your account on the website or mobile app. If you need to file a claim, go to the forms page and then select the appropriate claim form to download/fill out.
